did you format the usb using GUID partitioning scheme? this has been removed from Disk Utility recently, and is only visible when you chose to "VIEW ALL DEVICES" and then select the device itself for "erase". Certainly threw me! If you don't do this, you won't have an efi partition and cannot boot.
sooo.. the solution above sort of works. but it prevents instruments from executing elevated code, in consequence the time profiler does not work. What does work is the solution by mlg (many many thanks), which is a kext that makes nvidia drivers look as if they are signed. -> #181
