Cannot tell too many interesting facts ... I had to remove one pair of memory modules and the graphics card to boot into the firmware screen, then did all the known corrections there (including setting memory to XMP profile), then had to figure out how to correct the old Sierra config.plist...
Some trouble after upgrading mainboard firmware of Z170X, as I couldn't boot at all anymore.
After figuring out, everything else went "normal": USB and audio needed fixes.
Not yet overclocked – but benchmarks are only about 5 % weaker than they were on my Sierra install with 4,4 GHz.
On my GA-Z170X-UD5 TH:
On 10.11 everthing perfect.
On 10.12 no audio yet, USB mixed bag, everything else I tried so far works.
Figuring out dual custom OSX entry in Clover was challenging – so far I haven't finished with USB and audio (but audio didn't work on anything I tried.)
You mean the UEFI Shell (started in the Clover menu)?
If you are suspecting the GPU – pull it out and boot with IGPU.
Are you booting the HD from USB? But even if you do so – the entry stays in NVRAM when you have booted with it plugged in once.
Are you following the instruction exactly?
You NEED the HD entry of EFI partition in NVRAM (see bcfg).
You NEED the HD entry of OS X partiton (and any other partition you want to boot) in Clover plist (see my file post #38).

Start with a USB stick with complete EFI folder (including BOOT folder).
Copy that one to EFI Partition.
Reboot with that in EFI Partition (without USB Stick).
Reboot once again into Clover, go to shell.
Write "bcfg boot dump".
Remove everything except the entry with your EFI Partition, normally...
Okay ... after lots of researching, thinking and trial-and-error (at least 50 reboots) I figured it out:
1. delete all PCI boot entries with bcfg (as described in the first post by Anachronaut)
2. write custom boot entry in Clover config.plist for all partitions you wanna boot
3. disable scan...
I'm having this issue on all my Hackintoshes so far ... just deleting the BOOT folder in the EFI Partition did it all the time. Clover booted as normal, but I would't get excessive boot entries.

Tried new Version 2.5 of CodecCommander, changed plist to 15C50 (10.11.2).
(Edit: The latter seems senseless.)
Audio works now even after sleep.
Now I updated to 10.11.3 without any issues – even NVidia beta driver works perfectly with GTX 960.

In Clover plist I wrote 4 entries for the RAM slots, starting with "1" ending on "4".
But as the first slot has to be "0" (and the last one "3") the OS just adds the "0" as empty, resulting in showing 5 slots...

Moving the realtekALC.kext to S/L/E didn't make a difference – I just tried right now.
