Correction the correct clover version is 4769. This has been tried with 2 DVI monitors with the same result.

Tried these alternatives. Same result. I'm going back to the very start and rebuild the USB installer from scratch and then implement the suggestions above and give it one last try. Mojave is an order of magnitude more difficult that my prior Mavericks and Sierra installations.
