I can't recommend running a GTX 690 in a Hackintosh. I tried everything to make the card work properly but there were just too many problems with it under OS X. Under Windows 7 it is a fantastic card and excellent for gaming! :geek:
Ultimately I replaced the 690 with a 780 (non Ti version)...

Successful upgrade to 10.9.5 on my Asus Maximus 6 Extreme (Z87 w/4770K, GTX 780 and USB Audio).
My steps:

The crash (kernel panic) appears to be USB related. Have you plugged in any new devices recently?
Try unplugging the Bluetooth USB dongle and see if you still get the kernel panics on start up. It's possible that the Bluetooth USB dongle may have experienced a hardware failure.
Thankfully Wifi has worked flawlessly for me but something wicked started in 10.9.3 with Ethernet and continued in 10.9.4. Given how wide spread the issue is, there doesn't seem to be much discussion on actual root cause.
Disabling TCP segmentation didn't help my situation at all when trying to use either of the 3.x drivers. The Ethernet port would still hang on large file transfers. The only driver that seems to work properly on my rig after updating to 10.9.3 (and subsequently 10.9.4) is the AppleIntel1000e...
You need make sure you have the latest BIOS installed. This is critical as older bios versions will be problematic. Also make sure you have any over clocking turned off when doing the build to be sure the rig is actually stable during the build process.
I haven't created a guide specifically...
Some things to try:
1. If it's a desktop rig, try using a static IP address. DHCP makes sense for laptops and other gear that is mobile but it's not necessary for desktops (or other devices like printers which always stay in the same location).
2. You might also try rolling back to v2.4.14...
Glad to hear your issue got sorted.
Given how wide spread this is, it would be great to find out what the actual root cause is so it can be fixed. Using an old driver is really just a workaround but it does seem to solve the issue for now.

Just so the original purpose of this thread is not lost, v1.3 of this card does work out of the box with Mavericks for the majority of the people myself included. :headbang:
In the future MAYBE. This largely depends on if Apple releases a new Mac with the Z97 chipset. They most likely will but its speculation at this point.
I am on Z87 so can't really give you first hand details on Z97. Personally, I am holding off on doing a new build until there is native OS X support for Z97. :geek:
I am using an Asus Maximus VI Extreme on my primary Hackintosh rig. It has been good so far. I had to put the boot loader on the EFI partition though (there is an excellent guide posted on this site for doing so and its really not hard to set up).
