No you need to add them to the /EFI/Clover/kext/10.12 or /EFI/Clover/kexts/Other
Since your card isn't supported natively by macOS you might need to take all the extra steps for adding the FakeDeviceID for your GPU but it won't hurt to test.

check the table and you'll see your card is listed in the AMD7000Controller.kext (1st Gen GCN architecture) table.
are you using the FakeSMC.kext that came with the package? or are you using an older version of FakeSMC?

and one more thing? why do you need to have IGFX enabled? set your nVidia as primary graphic adapter in bios and disable Intel Graphic and see how it works.
Maybe, but there is another solution that you can try.
Create a folder on your desktop and name it "Plugins", copy all the sensors inside it and then right click on your FakeSMC.kext and select "Show Package Contents" copy the "Plugins" folder inside the "Content" folder and close the window...
It has the latest FakeSMC.kext and all the sensors + HWmonitor and it works just fine on my rig.
Your hardware is compatible, But in order to get the graphics card to work you need to:
1. Set IGFX as primary in BIOS
2. Connect your monitor to R7 graphics adapter and turn the PC on, in this case you'll lose boot up screen but you be able to boot into macOS Sierra using R7 display adapter...
