The replacement card was exhibiting the same failure until I removed and reseated the wifi/bluetooth card a couple times and then it all worked fine. No additional Kexts required. all functions working properly. Bluetooth working during boot so can be used at Clover volume selection. Thanks. - Scott

RealDirt: Maybe instead of just reformatting the drive, try deleting and re-creating the partition. Go with HFS+, Mac OS Extended (Journaled) even if it's an SSD. Let the macOS installer convert it to APFS. It seems happier doing so itself. - Scott

I just upgraded from 10.14.4 to 10.15. It took about an hour and went smoothly as described. The audio broke but I re-added the Lilu KEXT and rebooted and that fixed it. All is well.
