Problem solved. OpenCore "destroyed" the UEFI boot entries (checked with > bcfg boot dump at UEFI shell).
Removing wrong entries didn't fix it, but CMOS reset fixed my problem.

1. That list is very incomplete because it is only showing the cards that officially shipped in a real Mac.
2. Sounds like you do not have working NVRAM which is why setting the web driver is not sticking.
3. You card does not support ECC... so that tab is correct.

if you can boot ok with your USB installer, then you should mount the EFI from your USB, copy the EFI folder from within to your desktop
remove your usb stick
