System died, now prohibit sign

Surfing the web, then computer rebooted. When it rebooted, I get the prohibit sign. Was able to get to the recovery partition (?) and tried to run first aid (w/ and without original usb install disk) and it failed. Tried -v and I get the attached output. Tried to restore from a time machine backup file, that said it completed but same behavior. Am I looking at a bad hard drive? Should I try to reinstall fresh?

Does the USB work to boot into the system? If so boot then mount EFI partition. If the drive and the USB and drag the USB EFI to the drive folder to replace it.
 
No. When I boot from usb, clover gives the option to boot from the usb installer, recovery from recovery hd, FileVault prebooter from preboot, macOS install prebooter from preboot and recovery from recovery. If I choose either of the prebooter options, I get the prohibit sign.

Do you get the Mojave option in clover without the USB installed?
 
interesting little exercise. Ended up Reinstalling macOS10.14.1...then reinstalled apps/data/networksettings from backup. Then started the process to update to get to 10.14.16...all seems to be working correctly. Finally deleted Chrome/Brave, just in case.
