I sorted the BT/WiFi issue... I just plugged the adapter into a different PCIE slot and it seems to all be working just fine now. Thanks for all your guidance with this!
Sill having sleep/wake issues... although perhaps it's less likely now that it's USB-related. Including my latest EFI in case...
I decided that now was the time to switch my wireless card from the M.2 adapter to PCIE and when I did this, BT started working! But, now WiFi is not connecting... it won't turn on! I added HS07 and set it to internal using Hackintool.
I really don't understand what could be the issue with the...
1) BCM94360CS that I took out of my 2014 MBP - connected via M.2 adapter. I was actually hoping to change this to a PCIE adapter at some point.
2) 13.1
3) Noted. I'll play around with this. The KVM switcher is new for me, so maybe it's an issue. The water cooler I've had for a while and didn't...
Attached to HS13 is a KVM switcher.
Here are the issues I am currently experiencing:
No bluetooth: it can be turned on/off but does not discover devices nor is it being discovered. No airdrop, watch unlock, or any other function that uses BT. Again, I could have swore I had this working the...
Thanks. I've updated the port comments to exclude "#" just in case and I changed HS06, which is the USB header attached to my CPU cooler, to USB 2 (I think that's right... it's a 9 pin connector).
I'm attaching updated EFI and ioreg file
Ok so I've now used Hackintool to create USBPorts.kext and it DOES seem like my USB 2 ports are recognized, except for one of them, which I can live with if I have to.
I hope I've generated this kext correctly. It doesn't seem to have resolved sleep/wake or BT issues. One thing I should note...
I am having some issues with sleep/wake and Bluetooth, so I went about the process of mapping my USB ports with USBMap. At least one of the major issues I am encountering is that the companion USB 2.0 personality for my MOBO's USB 3.0 ports is not being recognized when I plug in a USB 2 device...
Bluetooth recently stopped working after 1 year+ of not making any updates to machine. BT can be turned on and off, but will not connect to devices that were previously found and will not find new devices.

I've removed unnecessary kexts that were in Kexts/Other. Disabled AppleAHCIPort.kext patch. Installed all kexts to /L/E. System appears to boot fine, except that WiFi is still off. Are there some that should be duplicated and also contained in Kext/Other?
New Debug attached
Model is BCM9460CS2. I hope that that is what you are looking for.
I removed patches that contained 4352 in the comments. Note: I don't believe I ever changed the contents of a comment... I was just noting that I was confused by the appearance of the numbers 4352 in the comments when the patch...
I am not familiar with how to boot without caches. I did use sudo kextcache -i /, rebooted, and used debug tool (files attached)
Edit: The plist file previously attached did not show Inject Kext set to Detect. This new one attached does.
