I had the same problem with my Titan X going to an LG 4k TV. It was stuck at 30hz. I found this patch to remove the pixel clock and now I get 4k 60hz with no issues. Verified working in 10.11.2 with latest nvidia web drivers.

I should have posted this before... but no one else had this problem so I thought it wasn't relevant. After I boot the installer partition and it gets to the DSMOS hang, if I press the power button it will KP (See Attached). I'm not sure if that is supposed to happen or not.

The only thing I can suggest at this point is to try booting with this kernel flag: UseKernelCache=Yes. If it doesn't boot you'll have to wait for a solution...

Yeah, deleting that kext on your SL drive makes it not bootable... good thing I was able to get back in through safe mode. But it doesn't work if you delete it from either drive... I just tried it.
mrmylanman you should try booting with npci=0×2000 as a flag... I herd it fixes the PCI Config problem.
On my build I've tried booting 32bit mode with no luck also... I'm sure Tony will come up with a solution for my problem (what ever it is), but I don't except it for a day or two... that dude...
