Have you set up your BIOS for OSX?
If your USB is for sure set up correctly, and your BIOS are set correctly, then it might be a hardware problem. Does everything else run as it should?
Hmm.... I read through the config, and it's pretty vanilla. Nothing looks wrong/out of place.
Maybe try pulling out your dedicated GPU and using your iGPU and see if you can get to the installer?
If that doesn't work - I wonder if the USB installer is damaged/incomplete.
Hmm... From what I've seen, when it crashes this early in a boot (see "Mac OS version: Not set yet"), it is usually a hardware issue. Often it's due to a CPU being incorrectly faked in Clover.

Hey bud, you'll want to add all your hardware to your profile - there are more parts in your rig than just your motherboard.
Also - it wouldn't hurt to list the "multiple bios options", as I'm not quite sure what you mean by that.
Try verbose booting, and take a picture where it hangs then...
IO port or Serial port? I'm not 100% sure if there's a difference, but I've never seen disabling an IO port referenced to fix this issue. I'm on Haswell though, so I might be wrong!
As for the prohibited sign - boot verbose and see what output you get, then take a picture of the whole screen...
By your image - you don't have either of the OsxAptioFixes installed?
You'll want to grab a copy of OsxAptioFix2Drv-64.efi and place it in the drivers64UEFI folder shown in your image, then see if you can boot further.
Hey man - that last line is "Still waiting for root device..." - make sure you're using a USB 2.0 slot (preferably on the back of your machine, connected directly to the mobo's back panel). If that doesn't work, try every USB port you have and see if you can get further.
Sounds like the EFI on your SSD is missing something that the USB has.
I'd double-check that you have that established. From a quick look through your hardware, the kexts you'd need are:
FakeSMC.kext - this one's an essential
RealtekRTL8111.kext - for LAN

Ahhh, the dreaded OsxAptioFixDrv-64.efi error - it looks like you're using fix 1. Have you tried using OsxAptioFix2Drv-64.efi?
