You could try changing it (with Multibeast) to iMac12,2 and see how you get on.
It's easy enough to revert if you don't like it.

appears i got it, did a fresh install of mavericks 10.9 to make sure.

for the install, changed enable internal graphics to AUTO, then after the install, changed it to YES, now auto/manual sleep i no longer get the black screen. hopefully this will help.

- Init display first: PEG
- Enable Internal graphics: Yes (even though I don't use it)
- Internal graphics standby mode: No
- Internal graphics deep sleep: No

not completely out of the woods... back to the basics, starting over, set to optimized defaults, ACHI and VT-D disabled...

No, although it happens less frequently, it still happens.
I'm running 10.9.2.
This past weekend, swapped motherboards to a GA-B75m-DS3H (which i have 2 running systems on 10.9.2. w/o any issues) and just last night, wake wouldn't come up correctly with the new mobo.
Same CPU, i7-3770
Same GPU, EVGA GTX 650 Ti BOOST 2GB.

Tried all tricks i could find mentioned everywhere and nothing seems to be the solid fix.

Going to try another graphics card and if that does the trick, getting rid of the GTX 650.... bummer.

I solved a similar problem the other day but disabling the Extreme Memory Profile (XMP) on my Z77-DS3H. In 10.9.2 the 'About this Mac' seems to manage to recognize the correct frequency of my RAM without XMP enabled, and it corrected my weird sleep issues.

BIOS is where you access the Extreme Memory Profile (XMP)? I think mine is whatever default would be for "Optimized Defaults"
I recall "About this MAC" reading the RAM freq correctly, but if you enable XMP, does "About this MAC" read it incorrectly? How far off is it?

It used to be (with Lion and Mountain Lion) that if I didn't enable the XMP to Profile1, my 1600MHz RAM would appear in About this Mac as 1333MHz. Setting XMP to Profile1 fixed the issue. In the latest version of Mavericks, however, the XMP was causing me to lose my USB and display after sleep. Disabling the XMP solved the issue and the frequency read on my RAM stayed at 1600MHz. I think it's a 10.9.2 thing.

I think that once you enable the XMP profile, THEN you can change it to Auto/Turbo/Whatever. That said, I'm on a Z77-DS3H. I'm not sure what your board's options are or if this is even your problem. It just happened to work for me.

here is something to try - I have found that certainly on the Z77X-UD5H Rev1.0 if you are using Mavericks the old advice for Gigabyte boards about setting XHCI Mode to Auto does not appear to be correct anymore.

I say does not appear because I have only tested on my own board so do not know for sure. On my board Smart Auto is what gives me full functionality and no issues with USB 3.0 - this I believe is down to Apple having changed the way that USB 3.0 is implemented in the OS.

Try changing XHCI Mode to Smart Auto and test to see if it makes any difference.
