I'm running macOS Monetery 12.3.1 (the latest version) on my Thinkpad T440p Laptop.
macOS runs fine but i keep getting this error:

Volume Hash Mismatch
Hash mismatch detected on volume disk1s5.
macOS should be reinstalled on this volume.

I have already fresh installed macOS but i still receive the error.

Sometimes it's just cosmetic but sometimes the system stops responding & i have to restart the Laptop.

Is there any fix/solution to this problem ?

boot into recovery & run the disk check stuff

Also you told us nothing about the setup.... i assume you are using ssd? how old is the computer?

I have already done something like running First Aid through Recovery mode but it didn't resolve the issue.

I used this Guide to setup my Hackintosh: https://github.com/valnoxy/t440p-oc

Yes, i'm using a SSD & in attachment you can find every detail about my Laptop.
