Dell T3500 second part of installation failing

Bios is updated to A17
Bios reset to factory default, then Drive mode changed to AHCI.

I was able to get to the installer and begin the Mojave installer. After the first reboot, I got to Clover and selected "Install macOS from macOS" (the SSD) as you normally would. However, I am unable to proceed. After a minute or two I get the prohibited symbol.

Installer USBs are just a straight forward Unibeast installer set to Legacy mode. I made two of them, one on a USB 2.0 stick, one on a USB 3.0 stick.


If I boot with the USB 2.0 stick, I get stuck in a boot loop with this screen (attached)

If I boot with the USB 3.0 stick, it hangs on the apfs_module_start:1393 line, and then later continues with what I believe says "waiting for root device" and the prohibited symbol.

I've now tried the following:

At Clover Boot Screen select Options > PCI devices > USB Ownership and USB Injection.
Added USBInjectAll.kext in /EFI/CLOVER/kexts/Other/
Added XHCI-unsupported.kext in /EFI/CLOVER/kexts/Other/
Added USB Port Limit patches (3) via Clover Configurator
Changed SMBIOS to MacPro5,1 via Clover Configurator

Tried 5 of the 8 USB ports so far. Some don't work at all now, the others behave the same as before. I am confused about one thing though. If I can get to Clover on USB, then why wouldn't macOS be able to see it?

Same results so far. I think this covers everything I've found by searching so far. Anything else I am missing?
