Your ioreg shows no bluetooth controller on the USB bus.
Make sure "fast boot" is disabled in BIOS.

Was able to test out Bluetooth and it does appear in Windows and it does function in Windows if I disable the stock Bluetooth adapter.

I had to erase the disk to install Windows so I will try to reinstall macOS and try it one more time.

I was able to reinstall macOS this afternoon, and after trying all of the steps detailed before, I have both Bluetooth and Wifi. However, System Report seems to show that it is using the stock Bluetooth adapter. I don't have a large enough drive to have Windows installed so I can't disable it there; should I follow the steps the OP described in their post to disable the stock adapter?

You should not have multiple bluetooth adapters.

Previously, I removed the stock Bluetooth adapter. During that time, the adapter built in to the wifi card would not appear, as you had mentioned. I got that same result on my initial run and when I first reinstalled macOS.

So, I reinstalled Windows. At first, no Bluetooth and the adapter didn't show up under device manager. So I reinstalled the stock Bluetooth adapter just to see the result. Both adapters would show up when the stock one was installed and I was able to disable the stock adapter and keep Bluetooth running via the card.

I get the same result under macOS after this third reinstallation. I can't seem to get the system to recognize card's Bluetooth adapter unless I have the stock adapter installed. However, the stock adapter is the one the system defaults to and takes precedent over the card. I tried repeating these steps by uninstalling the stock adapter physically and the system does not see the Bluetooth adapter in the card.

Probably a USB configuration problem.

Removing the irrelevant ports did the trick and now my computer uses the appropriate Bluetooth adapter.

Thank you Rehabman, John, and all of the others who have come before me. It took a little work, but I think I have a better understanding of macOS than I did before.

The card works in windows and not in Osx.
So i modified my config.plist to add Rehab man inject kext (like here https://www.tonymacx86.com/threads/readme-common-problems-in-10-13-high-sierra.233582/)
After i put FakePCIID.kext, FakePCIID_Broadcom_WiFi.kext, BrcmPatchRAM2.kext, BrcmFirmwareRepo.kext and USBInjectAll.kext in S/L/E with Kext utility.
It doesn't work, i don't understand what can i do now.

Please help me to make a diag.
