i used GTX 480 with 2 monitors. i changed to GTX 950 to use 3 monitors.

my problem is: 3rd Monitor (HDMI) doesn’t work.

Have you tried changing the system definition to iMac14,2 ?

One of the installation guide references recommends trying this when only one of two monitors is working with El Capitan. Maybe it would help you with your third monitor.
