Argh. I still get these weird extra boot entries. Problem not solved. I get an extra bogus entry for my boot disk after every reboot.
@Erwin: are you sure the problem is gone on your build?

Actually, it was MacMerlin who provided the info to solve the problem once and for all.

And don't forget to remove the entry that has \EFI\BOOT\BOOTX64.EFI at the end.

Could both of you provide screen shots?

You want screenshots of what exactly? The boot map in Clover?

Yes, the result of the UEFI command "map" and "bcfg boot dump" would be of help.

Does anyone have any random restarts? Sometimes if I playback video in Pr or launch say anything that uses the CPU my rig just restarts on it's own or locks up.

No sudden restarts on my rig. Try memtest and/or a cpu test program to see if there's a hardware problem.
I think I may just be pushing my OC too much. LOL

The only problem I seem to have is that when I boot, my left monitor needs to be turned off and then turned back on.
Could be an nVidia issue.

To my surprise, the problem is fixed. I only see my Clover EFI partition now in the BIOS, and nothing else, even after a couple of reboots.
Yesterday, I threw out all the spurious stuff in the EFI shell once more, disabled 2 erroneous boot entries in the BIOS, saved my BIOS Settings and now all looks fine... Weird. Let's hope it stays fixed. Don't see much of a pattern here.
 
I'm not having any random restarts.

I've been playing around with the OC options on my board (a z170x Gaming 6 - close enough to the UD5). After listening to Jerivalu's advice and reading through other forums I have OC'd my 6700k to 4.6Ghz, with the Cache ratio also set for 4.6Ghz. I used the "Enhanced Performance" memory setting for my Ballistix Sport 2400's - I have NOT OC'd that frequency yet. I've ran Prime95, xtu, and x264 benchmarks / torture tests. The highest voltage draw I've seen is 1.35v (average is 1.31v) and max temp of 68C. I followed Jerivalu's instructions and they worked ok - I would occasionally crash during xtu's benchmark even after setting vcore to 1.33. I reset to defaults and tried another forum's guide...reset again and tried Asus' guide. For the last day (after speaking with Gigabyte and voicing my concerns about heat and higher-than-necessary voltages) I have been running with the mobo's autosetting of 4.6Ghz for the 6700k. It seems to re running great - <70C and an average of 1.3125 are acceptable, right?
 
Sounds fine to me. In real life, the most demanding stuff I do is rendering 1080p movies in Final Cut Pro, with CPU temps in the lower 60's. The Noctua D15 does a great job.
What's your 32-bit Geekbench score? Overclocking RAM did very little for me, as far as I can tell.
