Finally got this to work after much trial and error.

My system -
Gigabyte GA-Z68P-DS3 Rev 1.0 (BIOS F9)
Core i5 2500K
32 GB PC1333 RAM
Gigabyte AMD/ATI 6850 video card (VendorID ATI 0x1002, DevID 0x6739, RevID 0x0000)
The card layout looks like this
DVI -- DP -- HDMI

Follow the BIOS configuration for Clover after you have booted into your HDD.

Note that the latest (as of 11/13/2015) Clover is r3320. When I built my Yosemite USB stick a while back (around the time when 10.10.1 was released) the Clover version was r2960 which is what I still have on my USB stick. No plans on redoing the stick with the latest Clover, I don't have a need to do that.

I used Tony's Standard plist and the system came up fine. Earlier versions of Clover for whatever reason couldn't give me a Framebuffer that'll give me dual screen no matter what Framebuffer injection method I used. Now, a full year after Yosemite is released I can finally use it on my Hackintosh. Do not use any ATI injector (at least for this specific card) as it just messes things up. The Clover initiated card id is natively recognized and uses the RADEONFrameBuffer. Dual DVI works out of the box.

I installed the FakeSMC and Intel nic KEXTs in /S/L/E instead of the EFI partition. Just my preference and I will say I didn't test putting them in the EFI partition. I included all the configs and kexts in the attachment. Null power management kext is included but I didn't use it on my HDD. You will need it if you screw up your config.plist and don't have it installed. Kernel will fail to load while waiting on the PM piece to start.

DSDT was downloaded from Tony's site. I didn't bother with SSDT since I'm having Clover generate the p-states but I will say I haven't installed the Intel app to check if power management really is working.

The hardest thing to get going was audio. I tried all manners of drivers from Multibeast to pristine AppleHDA+toleda's auto patch script to no avail. Toleda's patch kept complaining that the 889 is not supported which is weird (but I think it's a bug because it'll say 889 not supported even if I picked a different codec). Finally figured out the layout was wrong in the DSDT downloaded from here. The necessary layout-id is "0x01 0x00 0x00 0x00" while the DSDT from here sets it as "0x79, 0x03, 0x00, 0x00". Downloaded DSDT Editor, made the necessary changes, restored native AppleHDA.kext, deleted the patch strings from Clover config.plist and the realtekALC.kext from EFI partition. Rebooted and checked using IOReg and it now shows the correct layout-id. Ran toleda's auto patch and it now doesn't say 889 not supported. It auto detected the 889 codec, select yes to inject via Clover and yes choose 1 for layout (not 2!). Reboot and VOILA!

P/S - The config.plist is for reference only, don't copy and paste it in literally as it is. I have customized it to my monitor resolution, time-out, startup drive, etc...

If your keyboard doesn't repeat when you hold down a key, try this " defaults write -g ApplePressAndHoldEnabled -bool false "

Turns out Apple tried to be more like iOS and enabled those pesky pop-ups to allow choosing different variations of the key.
 
So now that I've been installing stuff, I noticed slow SSD performance on this machine. 20MB/s read/write speed which is pretty abysmal for a Samsung 840 PRO and 850 EVO. Got about 450-500MB/s when the system was running Mavericks. Did Apple screw up 6 series SATA support?
 
It was just a loose cable. Oh well, the mods moved it into "support" instead of locking the thread. If you guys can't find it in the install guide, don't blame me.
